Intensive Care Coordination (ICC)
Care Coordinators & Peers provide direct community based, non-clinical support and assistance to youth and families. Using the evidence based Wrap-around Practice Model, they assist and support youth and families to develop a sustainable team and an effective plan of care that addresses the child/family needs within the community setting. Catchment area-CT. Eligibility: Youth with serious behavioral health needs that would benefit from a community-based, family driven plan of care.
Full eligibility: Medicaid, commercially insured, and uninsured. Youth through age 17.
-Youth with serious behavioral health needs returning to the community from congregate care or other treatment setting (emergency department, psychiatric inpatient stay, PRTF, Solnit).
-Children who are at risk of removal from home/community in need of service coordination and support.
-Youth who frequently utilize emergency departments and/or inpatient hospital.
-Children and families in need of a community-based, family driven plan of care.
